Common Causes of Chronic Backache

  • Annular tears in intervertebral disc – When the outer part of the spinal disc tears, it can cause pain and discomfort.
  • Facet joint related pain – Degeneration or arthritis of the facet joints is a leading cause of chronic back pain. These joints can become inflamed and transmit persistent pain signals – often effectively treated with radiofrequency ablation (RFA).
  • Irritation of spinal nerves – Medial branch nerves that carry pain signals from the facet joints may become overactive, resulting in long-lasting back pain.
  • Degenerative changes of the spine – Age-related wear of spinal joints and discs can alter spinal mechanics and lead to ongoing discomfort, especially during movement or prolonged standing.

What is Radiofrequency Ablation

Radiofrequency Ablation (RFA) is a minimally invasive pain management procedure used to treat chronic back and neck pain caused by irritated or overactive spinal nerves. It is commonly recommended for patients with facet joint–related pain who have not achieved lasting relief from conservative treatments such as medication or physical therapy.

During the procedure, a thin needle-like probe is precisely guided to the targeted nerve using imaging technology. Controlled radiofrequency energy is then delivered through the probe, generating heat that disrupts the nerve’s ability to transmit pain signals to the brain. The nerve itself is not removed; instead, its pain pathway is interrupted.

The procedure is performed without the need for general anaesthesia, and patients are typically discharged on the same day and can return to normal activities within a short period. Pain relief may last up to two years, depending on individual conditions and nerve regeneration.

By targeting the source of pain rather than masking symptoms, radiofrequency ablation offers a safe, effective, and long-lasting option for managing chronic spinal pain.

How is Chronic Back Pain Diagnosed?

To determine the cause of your back pain, your doctor will take a detailed approach, gathering important information about your symptoms, medical history, and any activities or injuries that might have contributed to the pain.

Physical Examination:

Your doctor will assess your posture, movement, and strength to evaluate how the sciatic nerve and surrounding muscles are functioning.

X-ray:

An X-ray helps detect issues with the bones of the spine, such as fractures, bone spurs, or signs of spinal degeneration that may be putting pressure on the sciatic nerve.

MRI (Magnetic Resonance Imaging):

An MRI provides detailed images of the soft tissues, including discs, muscles, and nerves, helping to identify conditions like herniated discs, which can compress the sciatic nerve.

CT Scan (Computed Tomography):

A CT scan allows us to visualize the bones more clearly.

RFA of Facet Joints

In patients with backache from facet joint arthropathy, radiofrequency ablation to the facet joints denervates the medial branch nerve and brings about pain relief.

RFA of Disc

In patients with backache from annular tears, radiofrequency ablation to the damaged annulus denervates it and brings about pain relief. It also stabilises the disc and potentially seals the tear.

Non-surgical treatment options are often the first step in managing sciatica. These methods aim to reduce pain, improve mobility, and prevent the condition from worsening. Common non-surgical treatments include:

I first saw Dr Marcus Ling for sciatic nerve pain on my left leg. It was impacting my daily life as I could not sit or walk for long without feeling pain in my left leg. Had my cervical spine disc replacement surgery with Dr. Marcus Ling. Very confident doctor with Impressive surgical skills.
