What Causes Osteoporotic Spinal Fractures

Osteoporotic spinal fractures tend to occur in the elderly, especially those in their 70s and above. The cause of it is due to osteoporosis, which is a condition associated with a decrease in the quantity of bone mass. Though this condition can affect both genders, postmenopausal women are more commonly affected.

These fragility fractures are usually associated with low impact trauma such as falling in the bathroom etc. However, many of such cases can occur in the absence of any traumatic event.

Clinical Presentation of Osteoporotic Spinal Fractures

Back pain, especially at the fracture site, is the most common complaint. This pain can persist for up to two weeks and usually improve with bed rest or the wearing of a back brace.

In severe and chronic cases, some patients will notice they are hunched over and are shorter in height. This is due to the progression of the fracture which has led to the development of a kyphotic deformity.

Methods of Diagnosis

Physical Examination:

Your doctor will assess your posture, movement, and strength to evaluate how your spine and muscles are functioning.

X-ray:

An X-ray helps detect signs of fractures, degenerative changes, or issues in the bones of the spine.

MRI (Magnetic Resonance Imaging):

An MRI provides detailed images of soft tissues like discs, muscles, and nerves, helping identify conditions such as herniated discs or nerve compression.

CT Scan (Computed Tomography):

A CT scan allows us to visualize the bones more clearly.

Treatment Options

This condition can be initially treated conservatively with the wearing of a spinal brace and the consumption of some painkillers. Serial X-rays should be taken regularly to look out for possible worsening of the fracture.

If the X-rays show further displacement of the fracture or the backache cannot be controlled adequately with painkillers, a keyhole procedure called kyphoplasty can be performed. This procedure is usually performed in the operating theatre with the patient sedated. It involves the insertion of an inflatable balloon to jack up the collapsed vertebral body and then cement is injected into the fracture site to stabilise it. The pain relief the patient will experience is almost instantaneous.

In addition to treating the spinal fracture, it is important that we treat the underlying osteoporosis which is the root cause of the problem. Taking calcium and Vitamin D supplements is the easiest way to start tackling the problem.

Depending on how severe the osteoporosis is, there are several medications ranging from once-a-week oral medications to six monthly injections to choose from. We should also participate in some weightlifting exercises to build up our muscle mass as well as spend some time in the sun to increase our Vitamin D levels.

Treatment Options For Back Conditions

There are a variety of pain treatment options available, ranging from non-surgical methods to surgery, depending on the severity of your condition.

  • Physical therapy: A tailored physical therapy program can help you strengthen muscles, improve posture, and increase flexibility, all of which contribute to a stronger, healthier back.
  • Medications: Non-steroidal anti-inflammatory drugs (NSAIDs) or muscle relaxants can reduce inflammation and ease pain.
  • Steroid injections: For severe pain, corticosteroid injections can provide temporary relief by reducing inflammation around the affected area.
  • Radiofrequency ablation injections: Radiofrequency ablation uses radiofrequency energy to disable the sensory nerves in the joints that cause pain. Deactivating the pain-causing nerves brings about long-lasting relief from chronic backache.

In some cases, surgery may be necessary if other treatments have not provided sufficient relief or if you are suffering from conditions such as a herniated disc, spinal stenosis, or severe nerve compression. Surgical options might include:

  • Spinal DecompressionThis procedure involves removing part of a herniated disc (microdiscectomy) or part of a lamina (laminotomy) to relieve pressure on the spinal cord or nerves, which can help alleviate pain and improve function.
  • Spinal fusionFusing two or more vertebrae together to stabilize the spine.

I first saw Dr Marcus Ling for sciatic nerve pain on my left leg. It was impacting my daily life as I could not sit or walk for long without feeling pain in my left leg.
