What Causes Neck Pain

Neck pain is one of the commonest musculoskeletal complaints to visit a doctor’s office. The causes of neck pain are varied and can range from a simple strain to cervical spondylosis, which is degeneration of the cervical spine. Sometimes, neck pain can also be due to a whiplash injury because of a road traffic accident.

Neck Pain Diagnosis & Investigations

The investigations which we will order for the patients will depend on the clinical presentation.

In young and healthy patients who present with neck pain of a few days’ duration, usually no further investigations are required if the clinical examination is unremarkable. The clinical diagnosis is usually that of a neck strain, which tends to improve with time.

In patients who are involved in traumatic events like a fall from height, X- rays and CT scans are usually ordered to look out for any possible fractures or dislocations.

An MRI scan of the cervical is ordered if the patient complains of shooting pain down the arms or unsteady gait to look out for any spinal cord and nerve compression (Figure A). Such a clinical phenomenon is usually known as cervical radiculopathy or cervical myelopathy.

In elderly patients with cervical spondylosis, there will be a loss of intervertebral disc height and the presence of osteophytes (Figure B).

Diagnosing Neck Pain

Physical Examination:

Your doctor will assess your posture, movement, and strength to evaluate how your spine and muscles are functioning.

X-ray:

An X-ray helps detect signs of fractures, degenerative changes, or issues in the bones of the spine.

MRI (Magnetic Resonance Imaging):

An MRI provides detailed images of soft tissues like discs, muscles, and nerves, helping identify conditions such as herniated discs or nerve compression.

CT Scan (Computed Tomography):

A CT scan allows us to visualize the bones more clearly.

Treatment Options

In most cases of neck pain, it can be treated initially with a course of physiotherapy and painkillers. Most patients tend to improve with conservative management.

In patients who present with weakness in their upper limbs or unsteady gait due to compression of their spinal cord or nerves, surgery should be considered. The goal of the surgery is to relieve the neurological compression and bring about stability of cervical spine (Figure C).

General Pain Treatment For Your Back

There are a variety of pain treatment options available, ranging from non-surgical methods to surgery, depending on the severity of your condition.

  • Physical therapy: A tailored physical therapy program can help you strengthen muscles, improve posture, and increase flexibility, all of which contribute to a stronger, healthier back.
  • Medications: Non-steroidal anti-inflammatory drugs (NSAIDs) or muscle relaxants can reduce inflammation and ease pain.
  • Steroid injections: For severe pain, corticosteroid injections can provide temporary relief by reducing inflammation around the affected area.
  • Radiofrequency ablation injections: Radiofrequency ablation uses radiofrequency energy to disable the sensory nerves in the joints that cause pain. Deactivating the pain-causing nerves brings about long-lasting relief from chronic backache.

In some cases, surgery may be necessary if other treatments have not provided sufficient relief or if you are suffering from conditions such as a herniated disc, spinal stenosis, or severe nerve compression. Surgical options might include:

  • Spinal DecompressionThis procedure involves removing part of a herniated disc (microdiscectomy) or part of a lamina (laminotomy) to relieve pressure on the spinal cord or nerves, which can help alleviate pain and improve function.
  • Spinal fusionFusing two or more vertebrae together to stabilize the spine.

I first saw Dr Marcus Ling for sciatic nerve pain on my left leg. It was impacting my daily life as I could not sit or walk for long without feeling pain in my left leg. Had my cervical spine disc replacement surgery with Dr. Marcus Ling. Very confident doctor with Impressive surgical skills.
