What is Degenerative Spondylolisthesis

Degenerative spondylolisthesis is a spinal condition which the vertebral body shifts forwards in relation to the vertebral body below.

This phenomenon results in a reduction in the space of the spinal canal, leading to compression of the spinal nerves at that level. Patients with this condition tend to complain of backache as well as radiating pain and numbness down the legs. As the condition progresses with age, patients may report a reduction in their walking distance.

What Causes Spondylolisthesis

This is a degenerative condition and tends to affect patients in their 50’s and above.

The intervertebral disc undergoes degeneration initially and this will result in an increase in stress to the facet joints. The facet joints will then undergo degeneration, and this degenerative process will lead to instability and an eventual slip of one vertebral body over the other. This can result in compression of spinal nerves at that level.

Spondylolisthesis Clinical Presentation

Lower backache is the commonest presentation in patients with this condition. As the condition progresses, patients may complain of radiating pain or numbness down their legs.

Physical Examination:

Your doctor will assess your posture, movement, and strength to evaluate how your spine and muscles are functioning.

X-ray:

An X-ray helps detect signs of fractures, degenerative changes, or issues in the bones of the spine.

MRI (Magnetic Resonance Imaging):

An MRI provides detailed images of soft tissues like discs, muscles, and nerves, helping identify conditions such as herniated discs or nerve compression.

CT Scan (Computed Tomography):

A CT scan allows us to visualize the bones more clearly.

Treatment Options

Patients can be treated initially with a course of physiotherapy and painkillers. If their symptoms do not improve with conservative management or they experience any weakness of their legs, surgery should be considered.

Laminectomy and fusion surgery is usually the surgical treatment of choice to address the issues of nerve compression and instability of the spine (Figure B). This surgery can be performed in a minimally invasive manner and patients usually only need to stay for two nights postoperatively, hence ensuring minimal downtime

General Pain Treatment For Your Back

There are a variety of pain treatment options available, ranging from non-surgical methods to surgery, depending on the severity of your condition.

In some cases, surgery may be necessary if other treatments have not provided sufficient relief or if you are suffering from conditions such as a herniated disc, spinal stenosis, or severe nerve compression. Surgical options might include:

  • Spinal DecompressionThis procedure involves removing part of a herniated disc (microdiscectomy) or part of a lamina (laminotomy) to relieve pressure on the spinal cord or nerves, which can help alleviate pain and improve function.
  • Spinal fusionFusing two or more vertebrae together to stabilize the spine.

I first saw Dr Marcus Ling for sciatic nerve pain on my left leg. It was impacting my daily life as I could not sit or walk for long without feeling pain in my left leg. Had my cervical spine disc replacement surgery with Dr. Marcus Ling. Very confident doctor with Impressive surgical skills.
