
Cervical myelopathy is a degenerative condition that is characterised by numbness and clumsiness of the hands as well as gait instability. This is caused by spinal cord compression in the neck. The compression can be due to the presence of bone spurs or intervertebral disc herniation.
Patients affected by this condition tend to be in their 60s and above. The severity of cervical myelopathy progresses with time and surgery is usually the treatment of choice in most cases.
To understand the main causes of cervical myelopathy it is important to know the structure of the cervical spine.
The cervical spine has 7 stacked bones called vertebrae, labeled C1 through C7. The top of the cervical spine connects to the skull, and the bottom connects to the upper back at about shoulder level. As viewed from the side, the cervical spine forms a lordotic curve that gently curves toward the front of the body and then back.

Cervical myelopathy can be caused by various factors leading to pressure on the spinal cord in the neck:

As we age, spinal discs lose water content, becoming less flexible, more brittle and thinner. This makes them more susceptible to tearing or bulging even with minor strain or twisting.
This natural degeneration starts relatively early in life (often in the 30’s) but progresses over time.

Sudden force: A fall, blow to the neck, violent twisting motion, or rear-end car accidents resulting in whiplash.
Repetitive Strain and Poor Posture: Prolonged poor posture, such as hunching over a computer or phone, can strain neck muscles and contribute to pain.

When a disc’s interior pushes out, it can press on the spinal cord in the cervical area.

Bone spurs or thickened ligaments narrow the spinal canal, compressing the spinal cord.
A patient is suspected to be suffering from Cervical Myelopathy when he/she presents with the above-mentioned symptoms. To confirm it, an MRI scan of the lower back must be performed.

An X-ray helps detect signs of fractures, degenerative changes, or issues in the bones of the spine.
An MRI provides detailed images of soft tissues like discs, muscles, and nerves, helping identify conditions such as herniated discs or nerve compression.
A CT scan allows us to visualize the bones more clearly.
Patients can be treated conservatively when their symptoms are mild initially. Conservative treatment can be in the form of physiotherapy to strengthen the neck muscles as well as painkillers to alleviate the symptoms. As the condition worsens with time and walking independently becomes a problem, surgery is usually the treatment of choice.
Depending on the nature of spinal cord compression and severity of the condition, the surgery can be performed either from the front (Figure A) or the back of the neck (Figure B). The goals of the surgery are to decompress the spinal cord and to provide stability to the cervical spine.
